Elevate Membership Details

Fee: $74.98 Monthly

• Included Services: Once-daily use of one chosen modality: PEMF, Compression, Vibro-acoustic sound therapy, Red light therapy, Cold plunge, or Infrared sauna.

• Discounts: Members receive 50% off additional wellness services, 10% off aesthetic services, 10% off skincare products, and exclusive member pricing on IV services.

• Scheduling: Services are by appointment only and subject to availability. Members are encouraged to book in advance.

• Discounts Policy: Discounts apply to standard pricing only and cannot be combined with promotional offers or sales.

Level Up Membership Details

Fee: $149.98 Monthly

• Included Services: 12 credits per month for wellness services (1 credit per service). Available services: Cold plunge, Infrared sauna, Norma-tech compression, PEMF, Vibro-acoustic sound therapy, Red light therapy.

• Additional Services: After all credits are used, additional services are provided at member-discounted rates.

• Credits: Unused credits do not roll over and must be used within the membership period. Credits are void immediately upon cancellation.

• Discounts: Members receive 50% off additional wellness services, 10% off aesthetic services, and 10% off skincare products.

• Scheduling: Services are by appointment only and subject to availability. Members are encouraged to book in advance.

Glow+ Membership Details

Fee: $129.98 Monthly

• Included Services: One “Not So Basic Facial” per month with an optional dermaplane service at no additional cost. This benefit cannot be rolled over, transferred, or exchanged.

• Discounts: Members receive 50% off additional facials, 10% off medical aesthetic services (e.g., neurotoxin, HA fillers), 10% off skincare products, and member pricing on IV services.

• B-12 Injection: A monthly B-12 IM injection is included, subject to medical clearance by a Nurse Practitioner. If clearance is denied, no compensation or substitution will be provided.

• Limitations: Unused facials or B-12 injections do not roll over and are voided upon cancellation.

• Scheduling: Services are by appointment only and subject to availability. Members are encouraged to book in advance.

Replenish Membership Details

Fee: $129.98 Monthly

• Included Services: One IV therapy session of your choice per month. Additional IV therapies can be purchased at $99 each.

• B-12 Injection: A monthly B-12 IM injection is included, subject to medical clearance by a Nurse Practitioner. If clearance is denied, no compensation or substitution will be provided.

• Discounts: Members receive 50% off wellness services, 10% off aesthetic services, and 10% off skincare products.

• Limitations: Unused IV therapies or B-12 injections do not roll over and are voided upon cancellation.

• Scheduling: Services are by appointment only and subject to availability. Members are encouraged to book in advance.

Membership Upgrades and Downgrades

Members may request to upgrade or downgrade their membership tier at any time. Requests must be submitted in writing via email to optimize@optimizema.com or mail to Optimize Aesthetics and Wellness 540 Main St., Tewksbury, MA 01876. The following conditions apply:

Within the Three-Month Minimum Commitment Period:

    •    Members may upgrade to a higher-tier membership immediately, and the new tier will take effect on the next billing cycle. The upgraded membership will require a new three-month minimum commitment.

    •    Downgrades to a lower-tier membership are not permitted until the initial three-month minimum commitment period is fulfilled.

After the Minimum Commitment Period:

    •    Members may upgrade or downgrade their membership tier at any time. Changes will take effect on the next billing cycle.

    •    Any adjustments to membership tiers will not result in prorated refunds or credits for the current billing cycle.

Membership Terms:

1. Minimum Membership Duration:

   - The initial term of this membership is for a minimum of three (3) months, beginning on the start date listed below.

4. Payment Schedule:

   - Monthly dues will be automatically charged on the date of sign-up every month.

5. Cancellation Policy:

   - To cancel the membership and avoid charges for the upcoming month, clients must provide written notice before the monthly renewal date. 

   - Notice of cancellation must be submitted via email to Optimize Aesthetics and Wellness or in writing to the facility’s address or via email at optimize@optimizema.com

   - If notice is not received by the 1st of the month, the client will be charged for the subsequent month, and the membership will be canceled at the end of that month.

6. Automatic Renewal:

   - After the initial three-month term, this membership will automatically continue on a month-to-month basis until canceled by the client or Optimize Aesthetics and Wellness.

7. Membership Hold:

   - Members may place their membership on hold for a period of up to one month per year. 

   - Requests for membership holds must be submitted in writing or email and are subject to approval.

8. Facility Rules and Regulations:

   - Members agree to abide by all rules and regulations of Optimize Aesthetics and Wellness

   - The facility reserves the right to terminate the membership of any individual who fails to comply with these rules.

9. Liability Waiver:

   - By signing this contract, the member acknowledges that they have read and understood the liability waiver provided by Optimize Aesthetics and Wellness, and agree to its terms.
